Frequently asked questions

What is septoplasty?

Septoplasty is a surgical procedure performed to correct a deviated septum. The septum is a wall of bone and cartilage, which separates the two nostrils. A deviated septum or crooked septum occurs when the septum is shifted towards one side of the nasal cavity. This can cause difficulty breathing and reduced airflow due to blockage of the nasal airway. During septoplasty surgery, the septum is straightened and repositioned to the middle of the nose. This procedure may include cutting and removing parts of the septum before inserting them into the correction position. A septoplasty can be performed by itself or often it can be combined with a rhinoplasty. How To Treat Epistaxis With Cautery?

Nasal bleeding usually responds to first-aid measures such as compression. When epistaxis does not respond to simple measures, the source of the bleeding should be located and treated appropriately. Treatments to be considered include topical vasoconstriction, chemical cautery, electrocautery, nasal packing (nasal tampon or gauze impregnated with petroleum jelly), posterior gauze packing, use of a balloon system (including a modified Foley catheter), and arterial ligation or embolization. Topical or systemic antibiotics should be used in selected patients. Epistaxis (nasal hemorrhage) can often be stopped with cautery (sealing off a blood vessel using current or chemicals). Epistaxis may be due to bleeding from the anterior or posterior nasal passage. Anterior epistaxis may be treated by pinching the nares closed for 10 full minutes. If pinching is unsuccessful and the bleeding site is visible and localized, the next step is to use cautery, followed by nasal tamponade if cautery is unsuccessful or not indicated. Posterior epistaxis is managed quite differently so identifying the site of bleeding is critical. Most cases of epistaxis occur in the anterior part of the nose, with the bleeding usually arising from the rich arterial anastomoses of the nasal septum, Posterior epistaxis generally arises from the posterior nasal cavity via branches of the sphenopalatine arteries. In most cases, anterior bleeding is clinically obvious. In contrast, posterior bleeding may be asymptomatic or may present insidiously as nausea, hematemesis, anemia, hemoptysis, or melena. Infrequently, larger vessels are involved in posterior epistaxis and can result in sudden, massive bleeding. Dr. Manni Hingorani, is renowned ENT specialist in National Capital Region, had handled many such cases, Consult Dr. Manni Hingorani, for best clinical advise. What is Thyroidectomy? Why it is done? How it is done?

Thyroidectomy is surgical removal of all or part of the thyroid gland, which is located in the front of the neck. The thyroid gland releases thyroid hormone, which controls many critical functions of the body. A thyroidectomy may be appropriate for people who have a thyroid tumor, thyroid nodules or hyperthyroidism, which occurs when the thyroid gland produces too much thyroid hormone.Hyperthyroidism can be the result of an autoimmune problem, too much iodine in the diet, a benign tumor in the pituitary gland, too much thyroid medication, a swelling (goiter) in the thyroid gland or an inflammatory process. In Conventional Surgery, the surgeon makes a small incision in the skin of the neck as close to a natural crease as possible to reduce the appearance of the scar. The surgeon parts a thin layer of muscle to gain access to the thyroid gland, then removes one or both lobes of the thyroid gland as well as any nearby lymph nodes that may be affected by disease. The surgeon then returns the muscles of the front of the neck to their proper position and secures them in place. The skin is closed with sutures or glue. A newer technique involves accessing the thyroid gland through the mouth. This surgery leaves no visible scar since there is no incision on the outside of the neck. When the thyroid gland is surgically removed, the body still requires thyroid hormone to keep vital functions in balance. Thyroid hormone replacement therapy involves taking synthetic or naturally derived thyroid hormones in pill form. What is Myringotomy? Why it is performed?

A myringotomy is a procedure to create a hole in the eardrum to allow fluid that is trapped in the middle ear to drain out. The fluid may be blood, pus and/or water. In many cases, a small tube is inserted into the hole in the eardrum to help maintain drainage. A myringotomy may be done to restore hearing loss caused by chronic fluid build-up and to prevent delayed speech development caused by hearing loss in children,to place tympanostomy tubes, these tubes help to equalize pressure. It may also help prevent recurring ear infections and the accumulation of fluid behind the eardrum or to help treat an ear infection that is not responding to medical treatment, to take sample fluid from the middle ear to examine in the lab for the presence of bacteria or other infections. After the procedure, pain and/or pressure in the ear due to fluid build-up should be alleviated. Hearing loss due to fluid build-up should improve as well. What are Cochlear Implants for hearing? How does a cochlear implant work?

A cochlear implant is a small, complex electronic device that can help to provide a sense of sound to a person who is profoundly deaf or severely hard of hearing. A cochlear implant is very different from a hearing aid. Hearing aids amplify sounds so they may be detected by damaged ears. Cochlear implants bypass damaged portions of the ear and directly stimulate the auditory nerve. Signals generated by the implant are sent by way of the auditory nerve to the brain, which recognizes the signals as sound. Hearing through a Cochlear implant is different from normal hearing and takes time to learn or relearn. However, it allows many people to recognize warning signals, understand other sounds in the environment, and understand speech in person or over the telephone.Use of a cochlear implant requires both a surgical procedure and significant therapy to learn or relearn the sense of hearing. Not everyone performs at the same level with this device. The decision to receive an implant should involve discussions with medical specialists, including an experienced cochlear implant surgeon. What is FESS? How is microdebrider assisted FEES better than conventional FEES?

Nasal polyposis is often encountered in rhinology practice. Those who fail conservative management, a definitive surgery is essential to achieve sufficient ventilation and drainage of the affected sinuses by using either microdebrider or conventional instruments for functional endoscopic sinus surgery (FESS). Before the use of microdebriders, sinus surgery usually included the traditional use of curettes and forceps. This was a manual method for extracting bone and tissue from the nasal and sinus cavities.Depending on the type of procedure being performed, some microdebrider can come with pre-bent tips, which allows for easier access to difficult surgical locations (like in paranasal sinuses). Some tips also allow for 360-degree rotation, which allows for more accurate approaches to the tissue needing to be removed.Blades also can be adjusted with the microdebrider. Straight-edged blades are more precise and can be less traumatic than other blades. While serrated blades provide a better grip for the surgeon. These blades can have the speed changed to allow for tighter precision of cutting as well as to cut bone. Thus, there is a significant symptomatic improvement in cases undergoing microdebrider FESS.Also, Microdebrider assisted polypectomy is precise, relatively bloodless surgery though the precision depends on the surgeon's anatomical knowledge and operative skills. Dr. Manni Hingorani, is an expert ENT surgeon, and have handled many Sinus cases successfully, she can be consulted online by booking simple and easy online appointment on icareheal website or by simply downloading icareheal app.
